Bring a PDF into your practice account — an intake form, a referral, a care plan. No rebuild, no setup.

Type into the fields on your computer or iPad, or hand it over. No printing.

Send a secure link — it lands with the patient, theirs to keep.
The National Asthma Council pilot is free for participating practices — no card, no charges. You set up a practice account to bring your own forms, fill them on screen, and share them with the patient. The NAC asthma action plan you can fill and share free, with no account at all.
