Sending a questionnaire — send, fill, return

Pick a patient in your log, choose a questionnaire, and send it the way that suits — it comes back scored, ready for you to read.

Clinician

A questionnaire is a form you send to the patient. They fill it on their own phone — or on the clinic device — and the completed, scored result returns to your Home log. papersGP shows the score; you read it. It never diagnoses.

Send a questionnaire in four steps
1

Pick the patient in your log

Open Home. Find the patient — they arrive by referral (REF) from your clinical software, from New patient + beside the search, or they’re already a row from an earlier visit. On their row, tap Add form/workflow +.

2

Choose the questionnaire

The chooser opens with Forms, Questionnaire and Workflows, and a search box. Under Questionnaire, pick the one you need. For the asthma pilot that’s GINA Asthma Symptom Control; the library also carries PHQ-9, GAD-7, K10, DASS-21 and more.

3

Choose how they fill it — “How do you want to do this?”

SMS

Text it to the patient’s phone. A secure link — they open and fill on their own phone, no login. The everyday choice while they’re with you or in the waiting room.

QR

Show a QR code to scan. A code appears on your screen — no mobile number needed. It expires in 10 minutes.

Hand over

Hand this device to the patient. They fill it here; the screen is locked to them until you unlock with your kiosk PIN.

Patient portal

Send it to the patient to fill in. They complete it in their own time, in their portal.

4

It comes back scored

While the patient fills their part, you carry on. When they finish, their row updates in your log — tap View to read the captured answers and the Score. The score is the questionnaire’s own scoring; the reading and the next step are yours.
